PAYMENT INFORMATION

Mental health services are provided and billed by Desert Sage Health. Our providers participate in most of the major health insurance plans in the region. Below are the health plans we are currently accepting. If your plan is not listed, we will be unable to file a claim on your behalf. However, we can provide you with a form, called a Super bill, that you can submit to your insurance company to request reimbursements, which, if approved, would be sent directly to you. Please be aware that with any health plans, there may be deductibles and co-payments for which you are responsible.

Payment Policy

Payment, co-payments and deductibles are due at the time of service.

Appointments not cancelled within 24 hours result in a no show or late cancellation fee of $50. If there are two no shows in a row all future appointments will be cancelled until balance is paid. 

 

Returned checks result in a $50 fee. 

 

Super bills are provided on request.
